About Us

Discovery At Home is a 5 out of 5-star Medicare Certified Home Health Agency. At Discovery At Home, we provide Home Health & personalized care services for seniors in their home including Home Health Aides, Nursing Services, Occupational Therapy, Speech Therapy, Physical Therapy, & CAMP (Cognitive Assessment Memory Program). These services are typically covered by Medicare & can be accessed with an order from your doctor/health care provider.

Discovery At Home also provides private pay personized care services with Home Health Aides or Certified Nurse's Aides. Our Care Associates may help with daily activities such as bathing, personal care, running errands, dressing, transferring, incontinence care, companionship and so much more! Care Associates are available 7 days a week up to 24 hours a day to help with activities with you or your loved one to help ensure their safety while maintaining independence in their home.

CAMP
Discovery at Home is participating in the CMS approved Cognitive Assessment & Memory Program (CAMP) that merges digital therapeutics with application-based technology and AI to craft the first-of-its-kind evaluation & treatment for mild to moderate cognitive decline.

For those who qualify for the program, home health clinicians employ the CAMP Assessment whereby they discern the patient’s deficits through inspecting multiple cognitive domains including: Immediate Recall, Delayed Recall, Language, Auditory Memory, Visual Memory, Executive Function, ADLs, Processing. This program aims to aid in improving patients overall cognitive status.
